If you have any questions about our participation with your insurance plan, please call our office before scheduling an appointment.
Tricare Prime and Veterans Affair insurance plans will need authorization prior to your appointment. Please contact your Primary Care Provider to initiate authorization.
Our online scheduler allows patients to self-schedule up to 90 days into the future. If you are an established patient, you should see a “welcome back!” notice appear after validating your name and date of birth. If you do not see this even though you have been a patient of the practice, please call the office after scheduling your appointment to verify your information, before filling out the registration form which will be e-mailed to you.
Registration
After scheduling an appointment, you will receive a registration link from eDerm.
Login with the patient’s last name and date of birth. If you didn’t receive the email, please check your Spam Folder. If you still can’t find the email or can’t login, please call the office. Registration needs to be completed at least 1 week prior to your appointment.
New Patients:
Please arrive 15 minutes ahead of your appointment.
